A. ELECTRONIC MEDICAL REPORT FOR PATIENTS IN CENTERS FOR RESPIRATORY FAILURE – DAY CARE
The electronic medical report is filled in by the attending physician whose specialization is relevant to the condition. B. AT THE REHABILITATION CENTER YOU MUST PRESENT:
- An electronic medical report signed and sealed by the Attending Physician; if it is a Hospital referral the report must also bear the Hospital’s round stamp. Additionally, the electronic medical report must bear a signature, stamp and authorization date by the Supervisory doctor with the Greek National Health Service Organization
- A discharge note issued by the Hospital or the Private clinic (if following hospitalization)
- An information note, recent laboratory tests -ECG-, a postoperative X-ray (if applicable) – a brain CT/MRI
- Identity card or passport
